Warrant Officer (Navigator) Slade was the son of William Henry and Sarah Slade, husband of Ivy Georgina Slade, of Causway Green, Worcestershire.
He was 36 when his aircraft was shot down above the Netherlands and crashed in the village of Engelen.
He is buried in the Engelen General Cemetery, Noord-Brabant, Netherlands.
